Electronic copies of the forms that make up the Transfer Documentation can be downloaded from this web site. They are part of the Procedures for transferring custody of State archives. It may be necessary to read the procedures before completing the forms.
Complete the Transfer Documentation (Transfer Proposal Form, Series Proposed for Transfer Form, Series Information Form and Consignment List) and save them on your network or computer. They can be saved as individual documents or as one document. They can then be uploaded as one or more documents.
Important: Please save your files with names specific to your agency. This will reduce the chance of other agencies uploading files with the same or similar names.
To upload your documentation using the form below, please follow these steps:
- Complete the contact details and the description of the upload (for example: Transfer Documentation for State Records' Registered files sent as four documents).
- Use the Browse button alongside the File 1 box to navigate to where the Transfer Documentation is saved on your network or computer. Select your document and click on open.
- Select the Form type relevant to the file selected.
- As applicable, repeat Steps 1 and 2 for each of the remaining File and Form type fields. Do not select the same document in each File box as this will cause the upload to fail.
- Start the upload by clicking the Submit button.
Warning: While this facility will allow the sending of up to four separate files, users are warned that the uploading of multiple large files may take a very long time and that such transactions may not complete successfully. If your files are large (eg. in excess 1Mb each) you should consider uploading them individually and/or using a compression utility such as WinZip to reduce their size.
Please note: You may send all your separate files as a single zip file. If you choose to do this, use the File 1 box and select the Form type: "All files in a single ZIP file".
An Upload Status report is generated. If the upload is successful the report can be printed or saved as a record. If the upload was not successful it will usually indicate why it failed to allow rectification.
The Transfer Documentation will be considered in the usual way and a reply will be sent.
Upload Form
Fields marked * are required. For keyboard users the Accesskey "7" takes you to the first field in the form. Then use the "Tab" key to move forward and "Shift" + "Tab" to move back. The "Up" and "Down" arrows move between radio buttons.
